As a Patient Engagement Executive, you will be responsible for the following duties:

  • Actively engage patients to promote their participation in various programmes, such as Healthier SG (HSG), Chronic Disease Management Programmes (CDMP), diabetic screenings, nurse counselling, and community care initiatives.

  • Initiate outreach and follow-up calls with patients to coordinate appointment scheduling, issue timely reminders, collect feedback, and convey information pertaining to relevant programmes.

  • Provide support to partners and affiliated clinics in identifying eligible patients and defaulters, as well as in monitoring patient progress.

  • Assist partners and affiliated clinics in referring identified patients to appropriate community and social service providers.

  • Facilitate the logistical arrangements for deploying nurses to partner and affiliated clinics, while serving as the liaison between the clinics and nursing staff to ensure seamless coordination and deployment.

  • Ensure the accuracy of patient records and regularly update data within internal tracking systems.

  • Respond to patient enquiries and concerns with professionalism, empathy, and strict adherence to confidentiality protocols.

  • Support the preparation of reports, monitor key performance indicators (KPIs), and ensure adherence to Ministry of Health (MOH) documentation requirements.

  • Assist in any other duties assigned.

Requirements:

  • Possess at least a Diploma relevant field.
