A leading medical facility in Singapore seeks a dedicated Patient Facilitator to support new patients throughout their care journey. Responsibilities include coordinating care, conducting check-ins, and assisting with administrative tasks to enhance patient experiences. Candidates should possess a GCE 'O' level, be willing to learn, and have strong social skills for patient communication.
Job Description
New Patient Facilitation
- Before the consultation, call to confirm the appointment and remind patients to bring the necessary reports
- After the consultation, follow up with new patients to offer additional support, answer questions, educational materials and resources to ensure they understand their treatment plan and make informed decisions regarding their care
- Provide orientation and guidance on the next steps in their care journey (what to expect), including follow-up appointments, referrals, and billing.
Coordination of Care
- Serve as a liaison between patients, oncologists, nurses, and external healthcare providers to coordinate appointments, treatments, and follow-ups.
- Facilitate communication with internal departments, such as claims finance, and Etc to streamline care delivery.
- Coordinate external referrals to specialists, rehabilitation, or palliative care services as needed.
Patient Check-ins
- Conduct random check-ins with patients throughout the clinic to assess if they require any assistance or support.
- Gather feedback on the patient experience to ensure the clinic is meeting expectations and identify areas for improvement.
- Document interactions in patient case notes, ensuring accurate records of the support provided or issues addressed.
Patient Discharge Assistance
- Coordinate with healthcare providers to support patients at discharge, upon the doctor’s request.
- Offer additional support, such as helping patients arrange transportation or schedule follow-up appointments, to ensure a smooth discharge process.
Financial and Administration Assistance
- Guide patients in understanding and accessing insurance benefits.
- Assist with applications forms, claims processing, and other administrative tasks that may be challenging for patients and families.
Clinic Operations Support
- Attend to patients

Job Requirements
Candidate must possess at least a GCE 'O' level.
Willingness to learn new skills & knowledge
Computer literate and proficient in basic computer software
Work as a team player within a cohesive team in a fast-paced environment.
Social skills in communicating with patients.
Working hours: Mondays to Fridays (8.30 am to 5.30 pm) & alternate Saturdays (8.30 am to 1.00 pm)
