Qualifikationen

• Strong clinical knowledge in pharmacotherapy and medication management.

• Experience in outpatient or community pharmacy settings is advantageous.

Aufgaben

• Review and verify prescriptions for safe, accurate dispensing.
• Counsel patients on usage, dosage, administration and precautions.
• Perform medication reconciliation and identify interactions/allergies.
• Facilitate Medication Delivery Service, PILBOX and other collection services.
• Provide pharmaceutical care and respond to medication inquiries.
• Work with doctors/nurses to support optimal outcomes.
• Maintain documentation and comply with pharmacy policies/regulations.
• Support telepharmacy, tele-dispensing and video consultations.
• Ensure proper storage, stock management and safety practices.
• Contribute to quality improvement and patient safety initiatives.
