Job description

• Provide structured nurse counselling, chronic disease education, health screening, and mental health support through in-person and virtual consultations.

• Work closely with General Practitioners (GPs) to identify suitable patients and develop individualized care plans, including medication administration, self-monitoring, and lifestyle modifications.

• Conduct assessments of patients’ social, dietary, and lifestyle factors; perform procedures like eye and foot screenings, spirometry tests, and educate on chronic disease management tools (e.g., insulin use, inhalers).

• Assess mental well-being using standardized tools and escalate cases to GPs when needed.

• Assist with tracking clinical indicators, coordinate patient services with Primary Care Coordinators, and support ongoing quality improvement initiatives within the network.

Requirement

• Degree in Nursing

• Registered Nurse with the Singapore Nursing Board

• Specialist Diploma in Diabetes Education/Gerontology or Advanced Diploma in Chronic Disease Management is preferred

• Willing to travel to the northern and central parts of Singapore

• Willing to work on Saturdays, when required.
