Mobile Nurse
Fullerton Health is building the entire health & wellness value chain in the community. As part of the growth, we are seeking a mobile nurse to provide nursing & ancillary-related support for our patients. As a Mobile Nurse, you will be primarily responsible for delivering ancillary services to individuals primarily in remote locations, off-site from traditional healthcare facilities. Your role will involve providing comprehensive nursing care, conducting health screening & assessments, administering treatments, and offering healthcare education to patients in diverse settings.
Key responsibilities:
· Provide ancillary healthcare services to patients primarily in remote locations, including but not limited to homes, community centres, and other offsite settings.
· Conduct thorough health screening & assessments to identify healthcare needs and develop personalized care plans.
· Educate patients and their families about healthcare conditions, treatment options, medication regimens, and preventive measures to promote wellness and disease management.
· Implement health promotion initiatives and preventive healthcare measures to improve patient outcomes and enhance overall well-being.
· Maintain accurate and up-to-date patient records, including assessments, interventions, and outcomes, in compliance with regulatory standards and organizational policies.
· Collaborate with interdisciplinary healthcare teams, including physicians, therapists to coordinate patient care, address healthcare needs, and ensure continuity of care.
· Respond promptly to medical emergencies to administer CPR/AED if required.
· Participate in clinical service development and quality improvement initiatives to improve patient care outcomes.
· Undergo professional development activities to enhance nursing practice.
· Perform Ad-hoc duties as assigned by supervisor
Requirements:
· Singapore Nursing Board registered nurse with 1 – 3 years of nursing experience
· Valid BCLS and AED certification.
· Competent in Diabetic Retinal Photography screening, Diabetic Foot Screening, and Nurse Counselling techniques.
· Previous experience delivering healthcare services in remote or off-site locations and willingness to travel with mobile equipment is compulsory.
· Strong clinical assessment skills, with the ability to evaluate patient conditions accurately and provide appropriate interventions and treatments.
· Empathetic and compassionate approach to patient care, with a commitment to delivering high-quality, patient-centered healthcare services.
· Proficiency in technology, including electronic health records (EHRs), mobile healthcare applications, and other digital healthcare tools.
· Additional certification or training in relevant areas such as chronic care management is preferred.
· Previous experience in service / programme development is preferred.
