Job summary

IDHealth, an initiative of Happee Hearts Movement, seeks a Medical Social Worker to join an interdisciplinary team serving adults with intellectual disability and their families in Singapore.

You will assess biopsychosocial-environmental needs, develop and implement intervention plans, and coordinate care across health and social partners, including advance care planning.

The role emphasizes documentation, collaboration with external partners, and outreach to raise awareness among providers.

Qualifications

• Min Degree in Social Work
• At least 2 years of relevant working experience in social/healthcare sector
• Understanding of and ability to perform holistic assessments of the biopsychosocial-environmental issues facing PWID and their caregivers
• Ability to develop, implement and review intervention plans in a systematic and person centered manner
• Understanding of the health, disability and social needs of PWIDs and their families
• Understanding the resources available for PWIDs and their families
• Ability to work well within a team
• Ability to work well with external partners
• Good communication skills
• Competent in documentation
• Ability to conceptualise and develop clinical and operational materials for the purposes of clinical, developmental and collaborative work

Responsibilities

• Assess biopsychosocial-environmental issues facing PWID and caregivers
• Develop an intervention plan for issues identified for PWID and caregivers
• Implement interventions based on the plan
• Reviewing and monitoring the interventions and plans
• Supporting behaviours of concern and communication needs in PWID with caregivers and service partners
• Supporting PWID and family in advance care planning and end of life issues with service partners
• Working with the team to set goals for PWIDs and caregivers
• Working with external partners and service providers across health, disability and social settings
• Clear documentation of encounters with PWIDs, caregivers, families, service providers and volunteers
• Home visits, clinic sessions, teleconsults and regular meetings
