Medical Social Worker

Job Responsibilities:

1. Support elderly patients and their caregivers with:

• Psychosocial and financial support
• Casework and counseling services

2. To facilitate successful integration back to the community by:

• Collaborating closely with the multi-disciplinary team
• Implementing a comprehensive and coordinated care plan
• Implementing initiatives and support programmes that enhances the care and support of elderly patients and their caregivers within the community

Job Requirements:

• Degree and postgraduate diploma in Social Work.
• Must have an interest to work with the elderly in a healthcare setting
• Ability to communicate with vernacular languages speaking clients. (in order to provide psychosocial, financial support and assistance to Vernacular Languages Speaking Clients.)
