Medical Social Worker

Responsibilities

1. Provide social casework management, conducting psychosocial and financial assessment, counselling and recommending appropriate intervention in the management of patient care
2. Facilitate discharge care planning
3. Participate in multi-disciplinary team meetings and facilitate family conferences
4. Network with relevant organizations, social service agencies and individuals to activate social and financial resources to meet patient’s needs
5. Conduct home-visits
6. Organize and run psycho-emotional-social group activities for patients

Requirements

1. Degree in Social Work or Post-Graduate Diploma in Social Work preferably with experience in healthcare setting
2. Must be a team player who enjoys both hospital and community setting
3. Compassionate, responsible, positive, dedicated, diligent and independent
4. Good interpersonal communication skills
5. Strong interest in working with the elderly
6. Ability to converse in various local languages preferred
