Job description:


This role will lead service planning and development activities under the HMCD (Dementia) team, focusing on the development, implementation, and evaluation of new delivery models to better meet the emerging dementia care needs of the community care sector.

  1. Conduct landscape screening, and drive innovation of new delivery models to better meet the emerging needs and new trends in dementia care
    • Lead the review of the dementia care landscape to identify emerging trends and needs, through literature review, reviewing overseas dementia strategies/services and key stakeholder engagements.
    • Drive the engagement and partnership with stakeholders to identify opportunities for collaboration that is in line with the Community Mental Health Division's work-plan and the National Dementia Strategy.
    • Manage and engage the clinical experts and clinical team to provide clinical inputs to Ministries/agencies and other Divisions for the purpose of policy review and service planning.
  2. Conceptualise and implement new proposals including the evaluation plan for pilot dementia programmes and services
    • Identify, conceptualise and propose new services and initiatives that address identified landscape gaps and needs, which complement the Division's directives in the area of dementia care.
    • Develop proposals and seek appropriate funding sources for the implementation of pilot dementia programmes.
    • Lead the planning of evaluation of pilot service models to evaluate their relevance to the gaps and effectiveness of the interventions. This includes the review of the processes, outcomes and cost-effectiveness, providing recommendations to scale up successful models or make adjustments to the model to improve outcomes.
  3. Programme management for pilot dementia projects including programme review for sustainability
    • Support the HOD to oversee the implementation and evaluation of pilot services and projects.
    • Outreach to potential referrals sources and other stakeholders to raise awareness of new service models/projects, to increase uptake of services.
    • Review and propose capacity development activities to enable the successful implementation of pilot services and projects.
    • Guide the collection and analysis of data, evaluate its value and benefits, and explore sustainability plans.

Requirements

  • Bachelor's Degree in healthcare related discipline, with at least 8 years' experience in healthcare or community care sectors and 2 years in managerial capacity.
  • Experience in service development, programme implementation and evaluation is essential.
  • Prior experience in Dementia care or geriatric areas is preferred.
  • Able to work well with groups from diverse backgrounds.
  • Able to multi-task and possess strong analytical, problem solving and project management skills.
  • A self-starter with strong learning ability, who is resilient, adaptable and able to manage change and ambiguity.
  • Good interpersonal and communication skills.
  • Strong in proposal and report writing.
