Job description

Responsibilities

  • Work with the leadership teams of partner organizations to align goals, establish roles, and develop the sub-regional strategy and workplan to achieve deliverables for Phase 1 and 2.

  • Collaborate with centre managers and ground teams to implement the workplan, monitor progress, and address challenges.

  • Engage and build partnerships with community care providers, regional health systems, grassroots organizations, Social Service Offices (SSOs), Social Service Agencies (SSAs), and other stakeholders to support integrated care efforts.

  • Facilitate regular meetings and collaboration sessions among centre managers, ground staff, ICCP providers, and other service partners.

  • Support the development of Care Teams to assist seniors in navigating care options and services.

  • Oversee the implementation of the InterRAl Care Assessment and coordinate shared Long-Term Care (LTC) plans.

  • Manage Care Teams and ensure timely access to LTC services, adherence to standardized referral protocols, and continuity of care.

Requirement

  • Bachelor's degree in psychology, Social Work, Project Management, or a related field.

  • Minimum of 2 years' experience in case management, overseeing Active Ageing Centre (Care) operations, or working within the healthcare or eldercare sector.

  • Ability to work independently,

  • Effective in strategic networking and stakeholder engagement

  • Community-oriented mindset

  • Proven project coordination capabilities

  • Strong administrative skills and proficiency in Microsoft and PowerPoint
