Qualifications

Job description

Job Scope

We are seeking a Locum Physician who will manage our residents' medical conditions through regular nursing home visits.

  • Develop holistic care plans for residents and optimize their medical conditions and functional status
  • Engage residents to develop a care plan that is aligned with their preferences and goals of care
  • Titrate and prescribe medication for the clients
  • Ensure accurate documentation of medical records
  • Provide medical reports, memos, referrals if required
  • Provide phone consultations/teleconsultations for ad-hoc emergency cases
  • Conduct medication reviews for residents following hospitalizations
  • Sit in on the QAC committee
  • Conduct 6 monthly MDRs

Job Requirement

  • Possess a basic medical degree or postgraduate qualification that is registrable with the Singapore Medical Council
  • Full registration with the Singapore Medical Council
  • In active clinical practice and possess a minimum of 3 years' experience as Medical Officer
  • Hold an Outpatient Medical Service license
  • Possess relevant experience in home care services would be an advantage
  • Specialization in gerontology or experience working with the elderly
  • Passion to care for the elderly
  • Active in clinical practice IT skills in office/ clinical computer (PC) applications required.
