Job summary

Qualifications

• Degree in Nursing, Psychology, or a Health-related field.
• Experience in healthcare or other people-centric roles.
• Strong communication, teamwork, and patient-education skills.
• Willingness to perform on-call duties on a rotational basis.

Responsibilities

• Conduct patient interviews and perform preliminary health assessments.
• Coordinate investigations, clinic appointments, and follow-up care.
• Monitor medication adherence and provide patient counselling.
• Educate patients and families on self-care and post-transplant health.
• Maintain accurate patient records and support data reporting.
• Participate in quality improvement initiatives and research activities
