Job Descriptions:

  • Conduct home visits for palliative care patients upon referral.

  • Carry out comprehensive nursing assessments to evaluate patient needs.

  • Develop and implement individualized nursing care plans based on clinical findings.

  • Modify medications as needed to ensure optimal symptom management, in accordance with doctors’ instructions.

  • Collaborate closely with the Medical Director and outsourced physicians on patient care.

  • Educate caregivers and family members on essential nursing techniques to maintain patient comfort.

  • Coordinate referrals to external services, such as inpatient hospices, for symptom relief or respite care when required.

  • Identify patients or families requiring financial assistance and liaise with social workers or counselors.

  • Recognize cases needing specialized counseling and direct them to the appropriate professionals.

  • Support families by helping them prepare emotionally and practically for end-of-life situations.

  • Offer emotional support and bereavement care to families as needed.

Requirements:

  • Possess a Diploma or Degree in nursing.

  • Registered Nurse with valid registration under the Singapore Nursing Board (SNB)

  • Minimum 2 years of clinical nursing experience, preferably in palliative care, community nursing, or home care settings

  • Strong clinical assessment and nursing care planning skills

  • Able to work independently
