Summary

This role is based in East region.

The Home Care Coordinator plays a vital role in supporting the delivery and operations of home care

services. Working closely with the Staff Nurse and other team members, the Coordinator ensures the

smooth functioning of administrative processes, client care coordination, and service quality. This role

is essential in maintaining high standards of care and operational efficiency within the home care

setting.

Description

1. Client Admission & Discharge Coordination

  • Conduct intake assessments, including basic financial counselling and service agreement.

  • Manage the admission and discharge process for home care referrals, ensuring timely and accurate documentation.

2. Service Monitoring & Staff Rostering

  • Oversee the daily provision of personal care services to clients.

  • Coordinate staff scheduling, including rostering and leave management, to ensure adequate coverage and continuity of care.

3. Operations Support & Administrative Management

  • Provide day-to-day operational support and maintain comprehensive and up-to-date records for all clients, including filing, tracking, and updating documentation as needed.

4. Staff Development Support

  • Assist in the implementation of training programs and competency frameworks for care staff to enhance service quality and staff performance.

5. Client & Caregiver Engagement

  • Obtain regular feedback from clients and caregivers to assess satisfaction and identify areas for improvement.

  • Address concerns and escalate issues appropriately to ensure client well-being.

6. Custodial Care Support

  • Provide hands-on support in custodial care when required, ensuring clients’ comfort and safety.

7. Other Duties

  • Perform any other duties as assigned by the supervisor to support the overall functioning of the home care services.

Requirements

  • Diploma / Degree in any relevant discipline.

  • Good communication and interpersonal skills.

  • Team player who is organized and meticulous at work.

  • Proficient in Microsoft Office.

  • Good understanding of working with elderly.
