HIRING! Senior /Staff Nurse (Home Care) - 5 days Office Hours

Responsibilities:

• Provide compassionate, patient-centered nursing care, ensuring comfort and dignity for patients in their final stages of life.

• Monitor patients' conditions, adjust care plans as needed, and support families through the challenges of advanced illness.

• Work closely with doctors and the healthcare team to coordinate care and serve as a trusted link between patients, families, and medical professionals.

• Offer emotional support and guidance to caregivers, helping them navigate home care needs, equipment requirements, and coping strategies.

• Ensure continuity of care by keeping accurate records and following up on treatment plans.

• Administer medications, perform wound care, and deliver hands-on nursing support with sensitivity and respect.

• Participate in multidisciplinary meetings to enhance the quality of care for each patient.

• Maintain patient safety, confidentiality, and adherence to hospice care standards.

• Provide reassurance and assistance to families, creating a supportive and respectful environment.

Requirements:

• Diploma in Nursing, registered with the Singapore Nursing Board.

• Comfortable using healthcare IT systems.

• Passionate about palliative and end-of-life care, with a deep sense of empathy and service.

• Strong communication and interpersonal skills to support patients and their loved ones.

• Detail-oriented, collaborative, and able to remain calm under pressure.
