Responsibilities

• Strategy and Initiative Development: support research, development and implementation of initiatives that advance social-health integration and community care.
• Partnerships and Stakeholder Engagement: build and maintain strong working relationships with internal and external partners and facilitate stakeholder engagements.
• Project and Programme Management: support planning, implementation and monitoring of partnership initiatives and pilot programmes, track progress and identify risks.
