Job description

• Drive population profiling to inform system planning and care delivery, making it more systematic and targeted.
• Utilize population health data effectively to generate insights for strategic planning.
• Assist in developing and maintaining PHM profiling products.
• Support the consolidation and reporting of Population Health indicators and performance.
• Facilitate strategic planning for Population Health Data and/or Analytics User Group and Health initiatives.
• Assist with data and analytics requests related to PHM.

EDUCATION

• Bachelor's degree in Mathematics, Public Health, or Statistics preferred.
• Experience in population health or healthcare planning roles is preferred.
• Proficiency in Microsoft Office required; experience with data analytics and visualization tools is an advantage.
• Strong written, analytical, and communication skills.
• Good people management skills with the ability to handle multiple projects effectively.
• Ability to work independently and as part of a team.
