As a key member of our dynamic team, you'll drive innovative initiatives that bridge hospital and community care, improve healthcare experiences for residents in Singapore's western region. This role offers unique opportunities in healthcare planning, lead transformative projects, and work with diverse stakeholders across the healthcare ecosystem.

Key Responsibilities

Support the strategic planning and development of the integrated community care system by collaborating with operational and clinical teams to

  • Develop community care models targeting local needs and supported by international best practices
  • Design and implement innovative strategies to strengthen hospital-community integration.
  • Transform strategic plans into practical, actionable work processes that enhance service delivery.
  • Support project performance management and analytics
    • Develop and track key performance indicators for community care initiatives
    • Provide strategic insights and recommendations based on programme outcomes
    • Preparation of updates and present analytical reports to senior management to guide decision-making

Lead project planning and management

  • Lead and manage projects from conception to implementation including project plans, budget, resource planning, timelines and deliverables.
  • Drive cross-functional initiatives involving multiple internal and external stakeholders
  • Support in stakeholder engagement and build partnerships
    • Build partnerships and facilitate collaboration with key stakeholders e.g. NUHS institutions, government agencies, community care providers.
    • Lead strategic discussions and working groups, provide secretariat and administrative support to facilitate alignment of strategies and support execution of social health care initiatives.

    • Successful execution of any other duties assigned by supervisors and HOD.

Key Requirements

Bachelor degree in any discipline

  • Strong analytical abilities
  • Strong written and verbal communication abilities
  • Adept in people skills to foster buy-in and collaboration
  • At least 2 years of working experience
  • Knowledge and experience in project management will be advantageous
  • Experience in the Singapore healthcare system and/or knowledge of integrated care models will be advantageous
