Job description

The Job

This role is responsible to overseeing the Case Management and Afterlife Memorial Service and for our elderly members.

Responsibilities

  • Coordinate the collection, cremation, and burial arrangements of the deceased in collaboration with appointed external Funeral Directors.

  • Maintain a high level of sensitivity and professionalism while liaising with families and service providers.

  • Create, maintain, and regularly update accurate case files and the database of Afterlife Memorial Service (AMS) members.

  • Ensure proper documentation and confidentiality of all records.

  • Manage the AMS Hotline and serve as the first point of contact for reported cases.

  • Conduct timely follow-ups and provide necessary assistance and guidance to affected families.

  • Compile and prepare required supporting documents for submission to relevant government agencies.

  • Ensure compliance with regulatory requirements and timelines.

  • Prepare and submit monthly reports detailing AMS case activities and status updates for internal review and audit purposes.

  • Be available for after-hours and weekend case activations, based on a rotational standby schedule.

  • Provide logistical and operational support for the Charity’s events and community outreach activities as required.

Requirements

  • Minimum Diploma in Social Work or related disciplines.

  • At lease 2 years of relevant experience.

  • Mature, independent and compassion in eldercare.

  • Good understanding of Social Service and VWO settings.

  • Proficiency in relevant software applications for documentation and reporting.

  • Must possess own vehicle. Transport allowance will be provided.

  • Good command of spoken and written English.
