Working Days & Hours: 5 days Office Hours (any 3-4 days a week)

Location: Kent Ridge

Job Description:

1. Review each patient’s medication chart or prescription for the appropriateness of the drugs prescribed and to contact the prescribing doctor when an intervention is needed. The review has to ensure that:
1. All orders are clear and specific
2. Doses are prescribed within therapeutic limits
3. Any potential adverse reactions and interactions are identified
2. Fulfil all drug orders accurately and in a timely manner.
3. Ensure that all related costs and revenue are accounted for, and accurately recovered.
4. Dispense medications to patients, explaining the indications for drug therapy and instructions for administration.
5. Provide drug information and lectures to other healthcare professionals.
6. Delegate, oversee and review the work of assigned subordinates.
7. Ensure that all sales and supply of medicinal products comply with legal requirements.
8. Maintain records pertaining to all phases of drug receipt, supply, storage and distribution as required by the hospital and/or legal authorities.
9. Represent the department in various clinical committees and perform other job-related duties as assigned.
10. Review pharmacy procedures and policies periodically and implement schemes to improve pharmacy workflow.
11. Supervise performance, conduct appraisals, write reports and make recommendations for confirmation, promotion and training needs of subordinates.
12. Actively participate in Pharmacy education and development programmes.
13. Practice continued reading of journals, publications, etc for professional advancement.
14. Implement and adhere to policies, practices and regulations established by the department, hospital and external regulatory agencies pertaining to pharmacy.
15. Provide relevant training to pre-registration pharmacists and trainees.
16. Support the hospital by participating actively in various activities organized by the hospital, including QC, community projects, staff welfare and recreational activities.
