Join Our Team as a Medical Social Worker!

Principal Purpose

Your role will be crucial in ensuring that quality medical social work services are delivered in alignment with our vision, mission, and strategic objectives. You will focus on core skills for assessment and intervention to meet the needs of patients in primary care, providing comprehensive, coordinated, and continuing care that complements other health professionals and community partners.

Key Responsibilities

1. Clinical - Casework & Counselling

  • Provide person-centered case management and counselling support.
  • Engage patients and their significant others to identify and understand their needs.
  • Conduct bio-psychosocial assessments and formulate care plans collaboratively.
  • Manage complex cases under supervision, including crisis interventions.
  • Collaborate with multidisciplinary team members and participate in case discussions.
  • Initiate referrals to community partners to mobilize resources for patients and families.
  • Timely case reviews and completion of case recordings

2. Education, Research/Improvement and Administration

  • Attend supervision sessions with your clinical supervisor.
  • Participate in departmental projects and quality improvement initiatives.
  • Engage in continuing professional education activities.
  • Perform CARE duties during civil emergencies.

  • Keen interest in primary care medical social work
  • ️ Proficient in written/spoken English with the ability to speak local languages and dialects

  • Good communication and interpersonal skills

  • Willingness to work at different polyclinic locations from time to time

Preferred Qualifications & Experience!

  • At least 1-2 years of social work experience; experience in a healthcare and social services setting will be an advantage
  • Bachelor of Arts with Major in Social Work / Bachelor Degree in Social Work / Post-graduate Diploma in Social Work
  • A registered Social Worker under the Singapore Association of Social Worker's Social Work Accreditation Board
