Job description

To effectively supervise case workers and interns to deliver good results in case management, programmes, community outreach, respite carers recruitment, and publicity, including assisting the Director (Clinical Services) in the operation of Sanctuary Care Management.

KEY RESPONSIBILITIES AND ACCOUNTABILITIES

  1. Spearhead case movement through regular team meetings, case review sessions, etc.
  2. Identify at-risk situations, develop strategies and intervention plans to address child safety issues and wellbeing.
  3. Monitor and evaluate the effectiveness of case management and other support programmes and activities extended to respite carer families.
  4. Develop relevant Standard Operating Procedures (SOPs) for the department.
  5. Conduct in-depth assessments and produce written reports on the suitability of applicants for the role of respite carer, present findings to management, and follow up on decisions and recommendations.
  6. Match respite families with foster children based on factors such as CANs assessments, age, race, religion, and the foster parent’s capability to meet the child’s needs.
  7. Conduct necessary checks, including announced and unannounced home visits, to ensure respite carers can safeguard and promote the child's safety, stability, and wellbeing.
  8. Organise and participate in inter-agency efforts to support the welfare and development of foster children and young persons (CYP).
  9. Coordinate a 24-hour hotline for respite carers per duty schedule and ensure team follow-up on outcomes.
  10. Supervise and support foster placements to prevent breakdowns in existing care arrangements.
  11. Produce accurate case recordings and submit reports punctually to BT and other partners.
  12. Participate in roadshows and community outreach events as necessary.
  13. Meet regularly with the Head of SC/Supervisor to assess case progress and programme outcomes/effectiveness.
  14. Manage and support volunteers interested in providing services to SC.
  15. Contribute articles and social media content to promote mental health.
  16. Coordinate marketing and publicity projects to promote SC services.
  17. Support and conduct research related to practice.

Requirements

- Minimum 5 years of relevant supervisory experience.

- Experience working with children, CYP, and their families.
