Case Management Officer (Renal / Kidney Disease)

Job Scope:

• Responsible for the administration of the renal retardation programme which provides disease specific case management for patients with chronic kidney disease.

• Assess, plan, implement and evaluate care of patients with renal disease.

• Assist to conduct specialty nurse-led clinics for patients and their families where needed.

• Assist with overall coordination of renal replacement therapy and placement of patients in dialysis facilities.

• Provide patient care, education and counselling to patients according to the established standards of care and practice using patient care plans / clinical pathways.

• Work closely with physicians and other health-care providers in the delivery of patient care and services.

• Assist to review and develop patient education materials in consultation with Specialty Nurses, Nurse Managers, Doctors and Paramedical Staff.

• Precept newly joined renal coordinators in the delivery of specialty care where needed.

• Ad hoc duties as assigned by Head of Department.

Job Requirements:

• Minimum Degree, preferably in Nursing / Allied Health / Science or equivalent.

• Excellent interpersonal and communication skills.

• Preferably with working experience in a similar setting.

• Proficient in using MS Office.
