Job Scope:
Responsible for the administration of the renal retardation programme which provides disease specific case management for patients with chronic kidney disease.
Assess, plan, implement and evaluate care of patients with renal disease.
Assist to conduct specialty nurse-led clinics for patients and their families where needed.
Assist with overall coordination of renal replacement therapy and placement of patients in dialysis facilities.
Provide patient care, education and counselling to patients according to the established standards of care and practice using patient care plans / clinical pathways.
Work closely with physicians and other health-care providers in the delivery of patient care and services.
Assist to review and develop patient education materials in consultation with Specialty Nurses, Nurse Managers, Doctors and Paramedical Staff.
Precept newly joined renal coordinators in the delivery of specialty care where needed.
Ad hoc duties as assigned by Head of Department.
Job Requirements:
Minimum Degree, preferably in Nursing / Allied Health / Science or equivalent.
Excellent interpersonal and communication skills.
Preferably with working experience in a similar setting.
Proficient in using MS Office.
