Job description

Care Corner Seniors Services Ltd Caregiver Support Programme is an asset-based community development programme that aims to mobilize community resources to support caregivers. This programme complements formal case management services by providing both practical and emotional support to caregivers of seniors.

Key Responsiblities

  • Plan, implement and facilitate community programmes and group work sessions comprising caregivers of seniors and their support systems
  • Perform biopsychosocial assessments to identify caregivers' needs and provide necessary emotional support
  • Work with stakeholders such as Active Ageing Centres, Residents' Network, and non-profit organizations to recruit community assets and plan community programmes
  • Collaborate with hospitals and eldercare services to receive and manage referrals of caregivers
  • Maintain accurate and up-to-date records of assessments, interventions, and outcomes
  • Monitor and evaluate the effectiveness of support provided to ensure continuous improvement
  • Develop marketing materials for the programmes.

Qualifications

Bachelor or Post Graduate Diploma in Social Work with 3 years of post-graduate working experience in social service.

Requirements

  • Experience in organizing and facilitating community programmes and group work sessions.
  • Passionate about community work and caregiver support
  • Proficient in Microsoft Office.
  • High proficiency in written and spoken English with good facilitation and presentation skills.
  • Strong interpersonal and communication abilities with individuals and systems at multiple levels
  • Good analytical and assessment skills
  • Strong organizational skills and structured approach
  • Enjoy being part of start-up programmes and contributing towards their development
  • Able to work on alternate Saturday (half-day) to conduct community programmes
  • Willing to travel to conduct programmes and provide services in Marsiling and Toa Payoh.
