The Community Intervention Team (COMIT) supports individuals aged 18 and above who are experiencing mental health conditions, including dementia, as well as their caregivers. The team offers a range of services such as counselling, therapy, case management, psychoeducation, and assessments to better understand and meet the needs of each client.
Based on these assessments, the team develops personalised care plans or connects clients to more suitable services where needed. Caregivers are also supported through tailored interventions, including training, psychoeducation and support groups.
The role involves working closely with the team to provide holistic support to clients and their caregivers.
Responsibilities
- Manage a caseload of clients through casework and counselling.
- Conduct comprehensive mental health assessments and develop appropriate care and aftercare plans.
- Monitor clients' progress and adjust interventions or develop aftercare plans to support sustained well-being.
- Provide case management and psychosocial interventions tailored to individual needs.
- Offer basic emotional support and coping strategies to clients at risk of mental health issues.
- Conduct group work sessions to support client recovery and well-being.
- Deliver psychoeducation and mental health awareness to clients and caregivers.
- Plan and implement caregiver support initiatives, including training and support groups.
- Conduct community outreach and education programmes to raise awareness of mental health issues.
- Plan, implement and monitor the outreach, care process, referral and follow-up flow for the CREST service.
- Collaborate with partner agencies and organisations in planning and delivering mental health programmes.
- Monitor trends, gaps, and emerging needs in the mental health landscape to inform service planning.
- Engage and network with both internal and external stakeholders to strengthen support systems.
- Maintain accurate and up-to-date case documentation and records.
- Evaluate and report on programme outcomes and progress.
- Compile, analyse, and submit data and reports as required.
- Support other duties and special projects as assigned by supervisor or management.
QUALIFICATIONS
- A recognised Bachelor's degree in Social Work or Counselling.
- Candidates with a non-related first degree must possess a postgraduate diploma in Social Work or Counselling.
- Registered Social Worker with SASW or Registered Counsellor with SAC.
- Proficient in IT and comfortable with documentation and reporting systems.
- Strong interpersonal and communication skills to engage clients, caregivers, and community partners effectively.
- A team player who is also able to work independently with minimal supervision.
- Genuine interest in supporting individuals at risk or living with mental health conditions, including dementia.
- Knowledge and experience in Cognitive Behavioural Therapy (CBT) will be an advantage.
- Ability to speak additional languages or dialects (e.g., Mandarin, Malay, Tamil, or Chinese dialects) is preferred to better serve diverse client populations.
- Pleasant and empathetic personality, with a commitment to client dignity and empowerment.
