Responsibilities
- Identity presenting / underlying challenges and determine appropriate Advance Care Plan to enhance, maintain and promote psychosocial functioning of patients.
- Provide emotional support forEnd-of-Life Care, Critical Care, Grief and Bereavement to patients and their families.
- Lead patients and caregivers support group programs.
- Collaborate with other community service providers to provide appropriate services for continuity of care when patients are discharged.
- Maintain proper compliance documentation for case management and audit purposes.
Requirements
- Bachelor degree/Master in Social Work & or Social Science discipline.
- Minimum 3 years relevant experience, with specialization in Counselling and Psychotherapy.
- Registered with Singapore Association of Social Work & Registered Counsellor.
- Good clinical experience and Certified CTRT practitioners will be an added advantage.
- Passionate about serving elders.
- Resourceful, proactive, responsible, and team player with strong interpersonal, communication skills and patient-oriented personality.
- Mature and confident in dealing with patients, NOK and the public.
