What you'll be doing
Refine and implement integrated care models that support transitional, pre-, and post-hospital workflows to improve patient outcomes
Drive and support community-based projects that enhance care integration between hospital and community partners
Manage and analyse data to support evaluation, decision-making, and continuous improvement of programmes and services
Lead and participate in quality improvement and innovation initiatives to enhance the effectiveness and efficiency of community care services
Provide operational, administrative, and tactical support for community and home-based care teams
Coordinate and manage activities such as service counselling, scheduling, charging, billing, and other frontline operations to ensure smooth service delivery and positive patient experience
Engage and collaborate with healthcare and community partners, including primary care providers and nursing homes, to strengthen relationships and improve care delivery
Support community outreach and engagement through organising events, marketing programmes, and liaising directly with residents and stakeholders
Perform secretariat duties, manage department records, oversee budget and procurement matters, and ensure compliance with PDPA and relevant policies
Contribute to project teams and provide operational coverage or perform additional tasks as assigned by the supervisor
What we're looking for
Bachelor Degree in Healthcare Administration/ Business Administration or related studies
2 - 4 years of experience within healthcare operations/ community services
Strong project management skills with the ability to manage multiple initiatives concurrently
Passionate, resourceful, and outcome-driven in solving problems and delivering results
Able to navigate complex systems and build effective collaborations
Detail-oriented with strong planning and implementation capabilities
Critical and creative thinker with the ability to connect ideas and innovate
Team player with initiative, resilience, and the ability to work independently under pressure
