Job description

What you'll be doing

  • Refine and implement integrated care models that support transitional, pre-, and post-hospital workflows to improve patient outcomes

  • Drive and support community-based projects that enhance care integration between hospital and community partners

  • Manage and analyse data to support evaluation, decision-making, and continuous improvement of programmes and services

  • Lead and participate in quality improvement and innovation initiatives to enhance the effectiveness and efficiency of community care services

  • Provide operational, administrative, and tactical support for community and home-based care teams

  • Coordinate and manage activities such as service counselling, scheduling, charging, billing, and other frontline operations to ensure smooth service delivery and positive patient experience

  • Engage and collaborate with healthcare and community partners, including primary care providers and nursing homes, to strengthen relationships and improve care delivery

  • Support community outreach and engagement through organising events, marketing programmes, and liaising directly with residents and stakeholders

  • Perform secretariat duties, manage department records, oversee budget and procurement matters, and ensure compliance with PDPA and relevant policies

  • Contribute to project teams and provide operational coverage or perform additional tasks as assigned by the supervisor

What we're looking for

  • Bachelor Degree in Healthcare Administration/ Business Administration or related studies

  • 2 - 4 years of experience within healthcare operations/ community services

  • Strong project management skills with the ability to manage multiple initiatives concurrently

  • Passionate, resourceful, and outcome-driven in solving problems and delivering results

  • Able to navigate complex systems and build effective collaborations

  • Detail-oriented with strong planning and implementation capabilities

  • Critical and creative thinker with the ability to connect ideas and innovate

  • Team player with initiative, resilience, and the ability to work independently under pressure
