Job summary

Rural Roads Health Services - AHI at Alexandra Hospital, Singapore is recruiting a Social Work Coordinator. • Diploma in Social Work required.
• Good interpersonal and problem solving skills.
• IT knowledge and skills necessary for the job.

Responsibilities

• Provide administrative and operational support for the department, including managing the counter and related duties.
• Conduct intake interviews, assess financial needs, and assist with referrals to facilitate discharge planning.
• Support Medical Social Workers in case management and provide translation and patient escort support as needed.
• Participate in work process improvements and quality projects.
• Collaborate with other hospital departments and external organizations to enhance patient care and experience.
• Support departmental programmes and projects.
