Job summary

Recruit Express Pte Ltd is seeking a Diabetes Educator/Nurse in the West of Singapore to assess patients’ lifestyle and diabetes needs, provide education and personalised counselling, and educate on insulin injections and glucose monitoring.

You will review glucose profiles and support care plans, working closely with Endocrinologists and contributing to quality improvement initiatives within a permanent, on-site role.

Qualifications

• Valid SNB registration.
• Advanced Certification in Diabetes required.
• Experience in diabetes nursing or patient education.

Responsibilities

• Assess patients' lifestyle, dietary habits and diabetes management needs.
• Provide diabetes education and personalised counselling.
• Educate patients on insulin injections and glucose monitoring.
• Review glucose profiles and support individualised care plans.
• Work closely with Endocrinologists and support quality improvement initiatives.
