Job Description

• Responsible for providing a high standard of care through care management, assessment, planning, implementation, and evaluation of nursing care to meet the needs of clients, including educating family members.

• Responsible for delivering cost-effective care in accordance with established policies, procedures, standards of nursing care, and practices.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Primary Responsibilities and Duties (80%)

• Complies with approved standards of nursing practices.
• Assesses needs and delivers quality geriatric/dementia nursing care to clients.
• Performs pre-assessment of new clients referred by AIC.
• Ensures client enrolment complies with the Company’s SOP.
• Prepares documentation for submission to relevant government bodies.
• Assesses and coordinates with healthcare team to evaluate client conditions and implement care plans.
• Monitors, assesses, and reviews clients’ progression and adjusts care plans accordingly.
• Provides health promotion and counselling to clients and families.
• Collaborates with healthcare team for assessment, planning, and evaluation of client conditions.
• Performs nursing procedures such as medication administration and wound dressing.
• Oversees and manages nursing staff performance to ensure competency.
• Coordinates and monitors clinical and service quality indicators for continuous improvement.
• Performs CPR, AED, and first aid; actively participates in emergency situations.
• Supports health promotion activities within the Centre.

Secondary Responsibilities and duties (20%)

• Participates in clinical service projects.
• Engages in infection control campaigns.
• Assumes additional responsibilities as assigned by Senior Nurse Manager.
• Participates in staff training and development.
