Contract Healthcare Care Coordinator (5-5.5 days a week, West) #HYNT

We are seeking Care Coordinator to be outsourced to various healthcare organizations across Singapore!

• Building and fostering strong relationships with internal team members (care providers) and external stakeholders (patients and caregivers).
• Contributing to positive clinical outcomes by promoting preventive health screenings, reducing patient readmissions, and minimizing escalations to tertiary care.
• Gathering and addressing social determinants of health to positively influence both social and health outcomes.
• Utilizing technology to enhance patient empowerment, improve health literacy, and increase accessibility to relevant resources.
• Assisting in the coordination of patient care activities, using IT systems to track, document, and perform clinical indicators related to patient care.
• Conducting basic health assessments, closing gaps in preventive screenings, and ensuring patients are aligned with national health programs and initiatives.

Job Requirements:

• Min. O-Levels/N levels/A-Levels/Diploma
• Preference will be given to candidates with relevant clinic experience.
