Community Staff Nurse, Home Care (Office hours)

Job scope:

1. Assess and take comprehensive history of patients under her charge.
2. Perform physical examination of patients under her charge.
3. Develop and implement a plan of care.
4. Perform follow-up review of patients under her charge; coordinate and evaluate care using appropriate care plans.
5. Perform procedures within the scope of duties.
6. Provide specialised patient education and caregiver training.
7. Implement and coordinate individualised and group learning programmes with patients and caregivers.
8. Evaluate the effectiveness of patient education; perform health education and discharge teaching to patients, families, and caregivers.
9. Communicate treatment plans with patients and families.
10. Practise standard precautions when in contact with patients.
11. Assist in monitoring and upholding infection control practices, e.g., standard precautions and hand hygiene.
12. Educate patients and relatives on infection control awareness and practices.

Job requirements:

• Minimum Diploma in Nursing.
• Advanced Diploma in Geriatrics, Medical-Surgical, Chronic Disease Management, or Community Health is advantageous.
• Prior experience in Nursing home, Home care, or Medi-surgical ward preferred.
