As a Community Partnership Assistant Manager (Transitional Care), you will be responsible for the following duties:

  • Build trust and engage residents and stakeholders in Tengah New Town to identify community strengths, needs, and priorities

  • Conduct outreach through one-on-one conversations to understand community aspirations

  • Facilitate community-led initiatives to strengthen social connections and neighborhood well-being

  • Support residents in navigating health and social services to promote independence

  • Maintain partnerships with NUHS teams and external agencies to coordinate care

  • Facilitate case discussions between hospital and community partners

  • Train teams on strengths-based, community-centered approaches

  • Develop evaluation metrics for individual and community outcomes

  • Design community-supported discharge plans for high-need patients

  • Connect residents to local resources to reduce hospital readmissions

  • Monitor outcomes and improve post-discharge care processes

  • Prepare regular reports for the Tengah Hospital Committee

Requirements:

  • Bachelor's degree in community development, public health, social work or related field

  • At least 3 years' experience in population health or community development
