Community Outreach Associate

Responsibilities:

• Support the Manager/Team Lead in the day-to-day execution of activities at the Active Ageing Centre (Care).
• Conduct outreach and community screening for the community seniors residing in the assigned boundary.
• Build and maintain relationship/ rapport with community seniors.
• Support in planning and executing programmes based on seniors’ needs and interest to promote active lifestyles and social engagement.
• Gather seniors to join the planned programmes.
• Conduct befriending and buddying for the community seniors.
• Provide information and refer the community seniors to appropriate services as required.
• Assist community seniors on vital signs monitoring.
• Liaise with volunteers on programmes.
• Attend to walk-in, phone and text enquiries and correspondence.
• Track and report effectiveness for outreach and engagement, including attendance taking and report submission.
• Compile and submit required reports on a timely basis.
• Provide administrative support such as putting up documents for purchases, claims, and processing invoices received.
• Ensure confidentiality of the seniors’ data, in line with Organisation’s PDPA policy.
• Any other duties as assigned by the Manager/Executive.

Requirements:

• ‘O’ Level/ Diploma in any discipline.
• Good communication and interpersonal skills.
• Can-do positive attitude, with interest to engage the seniors.
• Team player who is organized and meticulous at work.
• Eye for details and meticulous in planning of events from start to the end.
• Willing to work on occasional weekends.
• Adaptable to changes.
• Ability to multi-task and perform under pressure.
• Able to work in a fast-paced environment.
• Proficient in Microsoft Office and data presentation.
• The candidates must be willing to travel between Senja and Woodlands.
