The Community Health Coach supports and empowers residents through personalised health coaching to embrace healthy lifestyle, manage their chronic disease conditions and improve quality of life. This includes working with residents on following-up with their health plan developed by primary care providers and community nurses, and facilitating their active participation in health-related programmes organised by community partners, such as Active Ageing Centres.

He/she shall participate and organise health education programmes and community health screening events.

Job Responsibilities

1. Perform health and social screening to identify health needs of residents.
2. Develop a personalised health plan to coach/support resident in achieving health and wellness goals.
3. Address challenges residents faced in achieving health promoting behaviours and lifestyle.
4. Document and maintain accurate records of residents’ progress in achieving their health goals.
5. Escalate care gaps/issues to community nurses.
6. Collaborate with community nurses or Community Health Managers to support residents in achieving their health plan and goals.
7. Assist residents in navigating the social and health systems.
8. Escalate complex health and social needs following the escalation matrix.
9. Provide basic Patient and Family Education for wellness and disease prevention.
10. Perform preventive health screening for residents in the Western region.
11. Participate in health promotion events and activities e.g. health talks.

Job Requirements

1. Diploma/Advance Diploma qualification or certifications relevant to healthcare industry is required.
2. Working experience in the healthcare industry is an advantage.
3. Good knowledge of various community services and support in the community setting will be an advantage.
4. Able to work in a team and independently.
5. Strong communication and social engagement capabilities especially with older adults in the community.
6. Strong organizational, coordination, and interpersonal skills.
7. Proficient in computer skills and Microsoft Office.
8. Able to converse in local dialects is preferred.
