Job description

You will be part of an integrated care team that delivers a person-centred care model for our seniors, including persons with dementia. Ideally, this person should also be promoting seniors' independence and maintaining their cognitive and physical functions. You will assist the Community Care Manager (CCM) in ensuring smooth operations of the Senior Day Care Centre (SCC).

Your job scope includes

Care Delivery
  1. Supervise and provide support to the direct care team on the delivery of Assisted Daily Living (ADL) tasks
  2. Coordinate and implement senior's individual care plan
  3. Supervise and maintain a safe and clean environment
  4. Conduct regular engagement with seniors to monitor their cognition and functional well-being
  5. Collaborate with healthcare professionals to track and report any concerns or issues
  6. Implement programmes as per guidelines and continually seek feedback to improve their effectiveness
  7. Tailor centre-based programmes, including intergenerational and volunteer programmes while adhering to the approved guidelines
  8. Support the preparation of relevant documentation to assist in incident reporting.
Stakeholder Engagement
  1. Support Next-of-Kin and Caregiver by providing information and guidance on the client's care.
Administration & Operations
  1. Maintenance of centre amenities.
  2. Transportation arrangements for clients
  3. Monitor inventory levels and submit inventory requests to Procurement or Facilities.
  4. Liaise with vendors, technicians and facilities management.
  5. Support the trialling and evaluation of new equipment or technology, including gathering feedback from users.
  6. Supervise and support the execution of crisis response Standard Operating Procedures (SOPs) by staff.
  7. Plan staff roster and manage day-to-day changes.
Quality Management
  1. Support the Community Care Manager in preparing and gathering documents for Internal and External Audits.
  2. Supervise and support the execution of new non-clinical SOPs.
People Management
  1. Supervise the delivery of activities by volunteers.
  2. Gather feedback on volunteers for re-engagement.
  3. Schedule training courses and on-the-job training for staff.
  4. Plan and schedule trainings.
  5. Conduct on-job training and assessments.

You should

  1. Possess strong interpersonal and communication skills, both verbal and written.
  2. Be able to work independently and at the same time, be a team player who is able to work cohesively with all levels.
  3. Be proficient in Google Suite Applications.
  4. Possess a growth mindset and embrace continuous learning and development.

It would be great if you

  1. Possess working experience in the hospitality sector or community support.

Qualifications

  1. Minimally a Diploma preferably in social service or related discipline.
  2. Experience managing and leading teams will be an added advantage.
