About the role

As a Community Care Associate (Day Care Centre) at Ren Ci Hospital, you will play a vital role in providing compassionate and holistic care to our elderly clients at our day care centre. This full-time position is located in the Central Region and will involve supporting the overall wellbeing and daily activities of our senior community.

BROAD FUNCTION

  • To facilitate and support all aspects of clients' activity of daily living (ADLs), taking into consideration of individual preferences.

  • To assist nurses or therapist to provide routine care for clients and to improve clients’ quality of life.

  • To support planning and implementation of daily programme at the day care.

  • To support any other non-clinical related tasks necessary for the daily operations of the day.

  • To perform daily work duties according to department or organisation standard operating procedures (SOP).

CARE DELIVERY

  • Meal and tea break preparation and serving.

  • Assist client in oral feeding.

  • Assist client in personal hygiene activities for example toileting, diaper changing, emptying urinary catheter / stoma bag, grooming and showering.

  • Delivery of NGT/ PEG feeding.

  • Perform medication assistance.

  • Monitor, and record of temperature, pulse and blood pressure and report any abnormalities to staff nurse or therapist.

  • Cognitive and behavioural monitoring and interventions.

  • Simple wound care management under the supervision of staff nurse.

PROGRAMME PLANNING AND IMPLEMENTATION

  • Support implementation of organization-wide programmes (e.g. volunteer activities, fund raising activities)

  • Contribute ideas and support implementation of site-specific programmes (e.g. daily day care activities, community outings) in collaboration with admin assistant and therapist.

INCIDENT REPORTING

  • Report/ escalate any client or staff related adverse incidents (e.g. falls, injuries, pressure ulcers, medication errors)

  • Support investigation of incidents

CAREGIVER, NEXT OF KIN (NOK) AND CLIENT SATISFACTION

  • Escalate complaints or disputes from caregivers, NOK, clients, to Reporting Officer (RO).

ADMINISTRATIVE DUTIES

  • Responsible for housekeeping, maintaining, and cleaning equipment at Senior Care Centre (SCC).

  • Purchase materials required for activities and submit claims for day-to-day purchases.

  • Support inventory monitoring.

  • Other duties as assigned by RO.

PROFESSIONAL DEVELOPMENT

  • Participates in relevant training programmes to acquire skills and knowledge that enhances work performance

ACADEMIC QUALIFICATION

  • Minimum GCE “O” or “N” level.

  • Relevant skills certificate from a recognized institution will be an advantage but not essential.

EXPERIENCE

  • Working in a geriatric set up, long term care or nursing home set up will be an advantage but not necessary.

  • Mature mid career individuals from any qualification level are welcome to apply.

  • Able to multi task and work in a stressful environment.

  • Able to maintain the honour, interest and dignity of the institution or organisation.

  • Able to serve those clients in need, regardless of race, nationality, religious or social status.

  • Compassionate and able to relate well with the sick patients of the organisation.

  • Caring, understanding, patient, empathetic and a good team player.

  • Cooperate with all colleagues towards the total health care of the community.
