Job description

You will be posted to Senja Care Home

Job Purpose:

As part of the Nursing Workforce for the Vanguard Senior Care, the Community Care Associate will assist the Centre Manager / Senior/ Staff Nurse in the delivery of required and optimal level (quality care) of healthcare services, based on the defined care model, to clients in the Vanguard Senior Care and residents in the community.

Job Responsibilities:

  • Attend to basic nursing and general care needs of clients
  • Conduct group activities for the clients
  • Ensure the safety, comfort and well-being of the clients
  • Work with the therapists on implementation of new activities
  • Work with nurses to maintain a safe and clean environment
  • Participate in quality improvement committee, and serve as a role model
  • Conduct home visits to residents in the community as part of the community outreach initiatives
  • Work closely with Community Outreach team and MSW to follow up and coordinate the care needs of community residents.
  • Provide support and involve in the planning and coordination of Active Ageing Programmes
  • Any other duties as assigned by the CM (VSC) and Senior/ Staff Nurse

Skills and Knowledge:

  • GCE "N" Levels
  • Preferably attained training in either of the following courses:

a) WSQ Higher Cert in Healthcare Support (Nursing Care)

b) ITE Approved Training Centre Healthcare (Home Care) program

  • Good communication and interpersonal skills.
  • Strong organisation abilities
