Qualifications

• Bachelor of Pharmacy from a recognized institution.
• Registered with the Singapore Pharmacy Council with a valid Practising Certificate.
• At least 2 years of relevant hospital/clinical/retail experience.

Responsibilities

• Provide safe, accurate, and timely medication dispensing in compliance with standards.
• Review, validate, and process medication orders to ensure safety and accuracy.
• Manage medication-related issues in collaboration with prescribers and teams.
• Conduct medication reconciliation during transitions of care to prevent errors.
• Perform medication reviews and optimize outcomes through pharmacotherapy.
• Counsel patients and caregivers on safe and effective medication use.
• Support chronic disease management with monitoring plans.
• Contribute to procurement, formulary reviews, and cost-containment initiatives.
