Qualifications

Job description

Duties

  1. Review medication orders for appropriateness, process drug orders, dispense medication to patients and supply drug stocks to nursing units.
  2. Counsel patients (inpatients/outpatients) on their medication.
  3. Provide drug information to doctors, nurses, and patients.
  4. Perform extension hours, night duties, and on-call duties as assigned by duty roster (if applicable).
  5. Assist in the preparation of ad-hoc items required on prescription orders.
  6. Adhere to legal and hospital policy requirements.
  7. Participate in continuing professional education (CPE).
  8. Assist in the training and supervision of pharmacy assistants and technicians.
  9. Participate in quality assurance activities (i.e. ward inspection/expiry checks) as assigned.
  10. Perform medication-therapy related duties (i.e. supply and distribution of controlled drugs, adverse drug reactions reporting, drug usage monitoring, etc.).
  11. Participate in medication reconciliation rounds and specialized counselling (warfarin and inhalation techniques counselling).
  12. Assist in providing clinical experience to pre-registration pharmacists.
  13. Maintain records relating to drug supply, storage, and distribution as required by hospital and legal regulations (i.e. JCI, MOH standards).
  14. Perform the function of purchasing pharmacist on a rotational basis (if assigned/if applicable).
  15. Assist in reviewing and updating procedure/protocols/intranet materials on a regular basis.
  16. Participate in approved pilot schemes/new services (if applicable).
  17. Represent pharmacy in various hospital projects as assigned by in-charges (if applicable).

Requirements

  1. A Degree in Pharmacy and registered with the Singapore Pharmacy Council.
  2. Experience in dispensing in an outpatient setting is preferred.
