Job description

We are looking for clinical counsellor to support case management of families of children with obesity in a school-based programme and as part of the work within the multidisciplinary Adolescent Medicine team to ensure seamless patient care.

Job Responsibilities includes:

  • Assist in monitoring the collection and input of data for clinical audit of service. Formulation and revision of guidelines, standards, and procedures. Proposing, redesigning and implementing strategies to improve clinical services.
  • Case management to provide and coordinate continuity of inpatient and outpatient care from initial triage to assessment, initiation of treatment till recovery or transition to adult services.
  • Coordinate patient care and providing updates to multidisciplinary team and initiate case review and discussion.
  • Provide counselling sessions.
  • Establish and maintain resources for psychoeducation of patients and families.
  • Establish rapport and build therapeutic alliance with patient and family for continuity of care across services and inter-sectors collaborations.
  • Work closely with patient and family and various services and coordinate discussions with community partners and schools. Conduct patient education and therapeutic groups.
  • Participate in research studies and quality improvement projects.
  • To co-faciliate, co-organise the various support and therapy groups, workshops, and other activities.
  • To Perform any other duties assigned.

Requirements:

  • Degree in Psychology/ Social Work/Counselling OR Postgraduate/Graduate Diploma in Psychology/ Social Work/Counselling with at least 4 years of relevant experience in in areas of adolescent health OR
  • Postgraduate/Graduate Diploma in Psychology/ Social Work/Counselling with at least 4 years of relevant experience in in areas of child and adolescent health
  • Good team player and yet able to work independently.
  • Strong interpersonal and communication skills.
  • Strong analytical, organising and prioritising skills.
  • Possess positive personality with high initiative and ability to solve problems and work with minimal supervision.
  • Work requires a broad knowledge of organization, communications, and program coordination.
