Clinical Counsellor - Assessment and Shared Care team (ASCAT)

The Clinical Counsellor (CC) supports SGH ASCAT Team parked under the Department of Psychiatry. The CC will assist the program director in the service planning and implementation of ASCAT and the role will have both clinical and administrative duties.

Program development and implementation
  • Assist program director in the following areas as listed:
    • Plan, implement and monitor the outreach, care process, referral and follow-up workflow.
    • Responsible for establishing the network with other stakeholders like community partners and/or related agencies to build collaborative partnerships and better facilitate care and support for clients and caregivers.
    • Establish and review work and communications workflow including identification, referral and escalation work processes as well as the development/update of the ASCAT service manual.
    • Provide supervision to the team to provide care and support, in alignment with the overall program objectives, adhering to the relevant ethical and legal guidelines.
Clinical
  • Help plan and execute direct training to community partners in mental health literacy.
  • Help with case management and care coordination for ASCAT clients.
  • Assist clinician in the assessment and triaging of new cases.
Staff Development
  • Assist the clinical lead in analysis and review of collated data and monitoring of the team’s progress and performance.
  • Assist the Clinical lead to oversee the training and professional development of the staff team with relevant competencies to provide the appropriate care and support for clients and caregivers, aligning to the intended care model of ASCAT. This includes ensuring all new/existing staff are clear on their job scope and responsibilities.
Requirement
  • Degree in Psychology / Counselling / Social Work.
  • Candidates with 2 to 3 years of working experience in relevant field will be an added advantage.
