You will carry out comprehensive client assessments, review intervention plans, monitor program effectiveness, and prepare timely reports for reviews and operations. Experience in social services is preferred, but all qualified applicants are encouraged to apply. This role is based in the East area with strong transport links and offers a competitive package

Responsibilities

• Carry out comprehensive client assessments and provide follow-up on decisions and recommendations.
• Review and evaluate case intervention plans on an ongoing basis
• Monitor and evaluate the effectiveness of case management, support programmes and activities provided to clients.
• Prepare and submit accurate reports promptly for operational needs, case reviews and discussions.
