Job description

A case worker in a youth home plays a vital role in supporting our residents by helping them navigate their emotional, social, and educational challenges.

Responsibilities

  1. Conduct assessments to determine the needs, strengths, and challenges of youth entering the home.
  2. Develop personalized care plans based on the needs of each youth, with regular monitoring and evaluation of progress, and adjust care plans as necessary to ensure the best outcomes.
  3. Offer crisis intervention and support to help manage difficult situations or conflicts.
  4. Work closely with various stakeholders.
  5. Attend meetings with other staff to discuss the well-being and progress of youth residents.
  6. Maintain accurate and up-to-date records of youth's progress, incidents, and interactions.
  7. Prepare reports for reviews, meetings, and other legal or administrative purposes.
  8. Connect youth to aftercare services when necessary as they transition out of the youth home.
  9. Perform any other ad-hoc responsibilities as assigned.

Requirements

  • Good knowledge and competency in case management support.
