Job description

Job Summary:

The incumbent will carry out basic case management work and apply intervention strategies appropriately, under the guidance of the supervisor, to improve the well-being of clients so that they may age in place in the community. He or she will also be involved in the centre’s community outreach and public education initiatives as well as support the centre’s volunteer management initiatives.

Duties and Responsibilities:

  1. Provides a person-centered case management services to vulnerable elderly living in the community which includes intake assessment, individualized care plans, home visit, basic counselling, care coordination, service linkage, and other clinical best practices relevant to promoting clients’ wellbeing.
  2. Work with volunteers to provide befriending and support to clients.
  3. Provide basic support services to caregivers to support their caregiving effort.
  4. Develop caregiver support network within the specified service boundary.
  5. Manage case documentation, home visit logs, and case statistics in timely and effective manner that complies with regulatory reporting standard.
  6. Develop, review, and implement appropriate intervention strategies in consultation with supervisor.
  7. Participate in case reviews, case supervisions, and case conferences.
  8. Participate in the care network and coordination with stakeholders within centre’s service boundaries.
  9. Assist in the outreach initiatives such as community mental health screening, health talk (when necessary), and promoting centre’s services to create public awareness of healthy aging.
  10. Perform other duties assigned by the supervisor.

Experience

  • 2 - 3 years relevant experience, fresh graduates may also apply

Special Aptitude and Knowledge

  • Passion to work in the community-based setting; possess good knowledge of healthcare and social services; have good organizational skills, is team-oriented with excellent interpersonal and communication skills; being resourceful and collaborative, ability to converse in various languages and dialects is an advantage.
