Job description

The incumbent will carry out case management work and implement appropriate intervention strategies under the guidance of the supervisor, aimed at enhancing the well-being of clients so they may remain resilient and age in place within the community. The role also involves participating in the centre’s community outreach and public education initiatives.

Duties and Responsibilities:

  1. Provide person-centered case management services to clients in the community, including intake assessments, individualized care plans, home visits, basic counselling, care coordination, service linkages, and other clinical practices to promote clients’ well-being.
  2. Engage clients and their caregivers to understand their needs, manage resistance to change, and collaborate with stakeholders to address complex cases.
  3. Conduct comprehensive needs and risk assessments independently for high-intensity cases.
  4. Implement care plans and interventions, evaluate processes and outcomes, and manage cases towards achieving service goals.
  5. Develop and evaluate client-centered care plans and strategies, including goal-setting, setting clear contracts, and engaging clients in problem-solving efforts.
  6. Participate in developmental programs and supervision to support personal and professional growth, aligning with agency standards.
  7. Collaborate with stakeholders in the care network to address individual cases and community needs.
  8. Engage in learning opportunities such as community practice meetings and professional development courses to enhance skills and knowledge.
  9. Maintain accurate and timely documentation, case files, and reports in compliance with regulatory standards.
  10. Assist in outreach initiatives like community mental health screenings, health talks, and promoting the centre’s services to raise public awareness of mental health and healthy aging.
  11. Perform other duties as assigned by the supervisor.

Job Specification:

Qualification: Bachelor’s Degree in Social Work, Psychology, Sociology, Gerontology, or Counselling.

Experience: 2-3 years relevant experience; fresh graduates may also apply.

Special Aptitude and Knowledge: Passion for community-based work; good knowledge of healthcare and social services; strong organizational skills; team-oriented with excellent interpersonal and communication skills; resourceful and collaborative; ability to converse in various languages and dialects is an advantage.
