A leading community service organization in Singapore is seeking a Case Management Worker to deliver person-centered case management services. The successful candidate will conduct assessments, implement care plans, and collaborate with various stakeholders to address client needs within the community. This role offers an opportunity to make a meaningful impact on clients' well-being and support healthy aging initiatives.
The incumbent will carry out case management work and implement appropriate intervention strategies under the guidance of the supervisor, aimed at enhancing the well-being of clients so they may remain resilient and age in place within the community. The role also involves participating in the centre’s community outreach and public education initiatives.
Duties and Responsibilities:
Job Specification:
Qualification: Bachelor’s Degree in Social Work, Psychology, Sociology, Gerontology, or Counselling.
Experience: 2-3 years relevant experience; fresh graduates may also apply.
Special Aptitude and Knowledge: Passion for community-based work; good knowledge of healthcare and social services; strong organizational skills; team-oriented with excellent interpersonal and communication skills; resourceful and collaborative; ability to converse in various languages and dialects is an advantage.
