Position Summary

The Case Manager I provides holistic, strength-based, harm reduction case-management services focusing on housing. Responsibilities include removing barriers to housing, assisting with appointments and documentation, developing housing plans, budgeting, linking guests to resources, and advocating for housing. The role involves visiting housing options, building landlord relationships, and participating in weekly housing match meetings.

Essential Functions

Case Management:
  • Provide timely, effective, holistic case management.
  • Maintain an active caseload and current case load tracking system.
  • Work with guests to remove barriers, create case plans, and perform needs assessments.
  • Meet with guests four times a month, enter weekly case notes, and assist with obtaining housing documents.
  • Facilitate weekly life skills groups on topics like budgeting and tenancy.
  • Use conflict management and crisis intervention techniques as needed.
Housing Navigation:
  • Develop individualized housing plans.
  • Visit housing sites and build rapport with landlords.
  • Maintain a list of current housing vacancies.
  • Assist guests in locating and securing housing through outreach and advocacy.
  • Coordinate guest move-out and develop exit plans.
  • Train new staff, interns, and volunteers as needed.
Record Keeping:
  • Maintain documentation, including housing documents, goals, and files.
  • Ensure timely entry of case notes into databases.
  • Manage guest exit procedures and billing accuracy.
  • Communicate guest status and challenges regularly with supervisors.
Miscellaneous Duties:
  • Participate in meetings, trainings, and community events.
  • Maintain positive relationships with staff and partners.
  • Perform other duties as assigned.
Leadership Core Competencies

Working Conditions

Requires walking, standing, bending, lifting up to 40 lbs., and operating office equipment.

Minimum Qualifications
  • Understanding of The Salvation Army's Christian mission.
  • BSW or equivalent degree preferred.
  • Minimum 1-year experience in case management with vulnerable populations.
  • Experience with HMIS preferred.
  • Experience with tenant/landlord relationships preferred.
  • Good physical and mental health.
Skills, Knowledge & Abilities
  • Valid driver's license and clean MVR.
  • Bilingual in English/Spanish preferred.
  • Effective communication and adaptability skills.
  • Proficiency in Microsoft Office and database management.

Pay Rate: $25.00-$27.00/hr.

Additional Qualifications

  • Experience with tenant/landlord relationships, HMIS, and case management preferred.
