Job Description & Requirements

Working as part of the Community Services team, the Case Worker is responsible for providing comprehensive care and support for complex and chronic cases that require assistance from multiple agencies. They achieve this through appropriate information sharing and referrals. This service complements the work of the Vincentian volunteers by offering casework and case management aimed at improving the quality of life for our Friends in Need (FINs). This role involves building strong relationships with various stakeholders and actively networking and collaborating with social service agencies across government, private, and community sectors.


Job Description

  • Conduct casework and counselling services to clients and their families, including gathering information, conducting psycho-social assessment, identifying common needs and goals for intervention
  • Provide Information & Referral (I&R) to link clients and their families to appropriate services
  • Able to pilot and lead new programme/ group work for clients independently
  • Maintain accurate and up-to-date case records, progress notes, and statistical data
  • Adhere to confidentiality and data protection policies and procedures

Requirements

  • Diploma/Degree in Social Work/Psychology/Counselling or related discipline.
  • 1-2 years of relevant working experience in Social Service Sector. Fresh graduates in the relevant field of work are welcome to apply.
  • Working knowledge of IT applications like MS Office.
  • Strong team player with good interpersonal and communication skills.
  • Teachable heart and willing to embrace new learning opportunities.
  • Must have the flexibility to work on weekends and weekday evenings (as needed).
