Job description

Job Purpose
The main objective of Case Management Officer (CMO) is to provide a continuum of psychological care, promoting optimal quality of life for women affected by mental disorders. This includes ensuring that patients receive comprehensive, individualized care and coordinated access to services. He/ She will need to collaborate with a multi-disciplinary team of doctors, psychologists, nurses, medical social workers, and other allied health workers in providing timely and appropriate treatment for the patients.
This role provides fulltime service to Women’s Emotional Health Service+ (WEHS+) / Promoting Parental emotional health to Enhance child Learning (ProPEL) based at NUH and some assessments or intervention may be performed in patient’s homes.


Duties and Responsibilities

  • Provide clinical and psychosocial assessments of patients referred to WEHS+/ ProPEL, ascertaining diagnosis and clinical goal planning.
  • Screen and Monitor patient’s mental status.
  • Provide counseling, psycho-education and advocacy for patients and families.
  • Assessment of symptoms and use of scales, monitor drug adherence, provide relapse prevention advice. Provide crisis assessment and intervention.
  • Formulating plans of patient care in collaboration with clinicians (specialist and the Consultant in charge) with reference to the clinical guideline for each sub-specialty clinic.
  • Ensure patient’s diagnostic tests, treatments, consultations take place timely Integrate and coordinate multiple discipline services or resources (i.e. FSCs, CRSS, Feiyue, Mother and Child Centre etc).
  • Plan and arrange for home visit date, time and venue with patient/ family. Carry out home visit on planned date and time.
  • Provide home intervention and psycho-education to patient and family in response to needs identified at the home visit.
  • Update the team during MDT meeting the findings of the home visit and follow-up on action plan agreed in the meeting.
  • Continue to support the patient/ family through home visits and phone calls until the team approved to discharge the case from programme.
  • Work with physicians and nurses to ensure suitably stable patients are timely and appropriately sited to community care based on given clinical/administrative criteria.
  • Attend to patient enquiries regarding treatment and referrals.
  • Organize and promotes patient education. Make sure education materials and information are up to date.
  • To be familiar with the community resources, maintain a list of available resources for the area near to the patient, and keep the list updated.
  • Point of contact and liaison between patient, family and hospital. Provide liaison service to the O&G Department, MSW, community partners.
  • Initiate and coordinate discharge planning for patient.
  • Perform data management, collating and maintaining of patient database for clinical and research purpose.
  • Conduct research and perform literature reviews for research projects. Prepare interview questions, collect data, write reports to summarise findings. Prepare materials for DSRB submission.
  • Assist in tracking, maintaining and reporting of quality clinical indicators and provide statistical report and updates on a regular basis.
  • Participate in projects related to case management and clinical pathway.
  • Perform other duties and responsibilities, as necessary, in the overall operation of the clinic.
  • Adherence to best-practice guidelines and evidence-based practice.

Job Specification/Requirements

  • Degree in Psychology.
  • Preferably has working or volunteer experience in the social sector with good people skills.
  • Experience working in mental health institute in inpatient or outpatient setting will be advantageous.
  • Knowledge of research methods.
