Overview

The case management officer will stay in close communication with internal and external stakeholders as well as patients across the care continuum to ensure care coordination, efficient care delivery, and removal of barriers. S/he will coordinate and assist the clinical and nursing teams in planning and logistical needs for patients requiring stem cell transplantation or cellular therapy. S/he will also provide administrative support, identify areas for improvement, and participate in quality improvement activities for the Haematopoietic Stem Cell Transplantation and Cellular Therapy Program.

Job Responsibilities

  1. Primary Responsibilities and Duties (80%)
  2. Support multidisciplinary team members regarding patient care matters.
  3. Act as liaison officer, receive, and coordinate new internal and external patient referrals.
  4. Conduct related and unrelated stem cell donor searches, facilitate donor identification, and communicate updates with the team.
  5. Coordinate appointments for donors and patients throughout various phases of the stem cell transplantation care journey.
  6. Ensure all necessary investigations are ordered in the clinical system at appropriate time points.
  7. Follow up to ensure all medical investigation results are generated promptly.
  8. Prepare documents required for stem cell transplantation care.
  9. Assist patients and families in navigating different services and locations.
  10. Provide education to patients and families regarding the stem cell transplantation journey.
  11. Communicate effectively with stakeholders to ensure smooth care transitions.
  12. Follow up with patients via phone when necessary.
  13. Administrative Support
  14. Perform internal documentation audits.
  15. Participate in audit preparation for the program.
  16. Serve as secretariat for related meetings of the program.
  17. Maintain and update databases.
  18. Operational and Quality Improvement
  19. Participate in quality improvement activities.
  20. Identify areas for operational efficiency improvements.

Secondary Responsibilities

  1. Provide cross-coverage for the Clinical Research Coordinator (Cellular Therapy).
  2. Perform other duties as assigned by the Reporting Officer.

Requirements

  • Bachelor's Degree in Nursing, Allied Health, or related Science field.
  • Experience in a similar setting is preferred.
  • Good interpersonal, teamwork, and communication skills.
  • Proficient in computer skills, including Microsoft Word.
  • Ability to multitask and work under stress.
